Medical History
For your protection we must know the condition of your health before treatment is started. Please be especially careful in providing answers to our medical questions.

Comprehensive Exam
The new patient exam is the cornerstone of our relationship. Dr. Blashford will answer any of your questions concerning dental health and will look for jaw joint concerns, oral cancer, bite difficulties, gum disease, tooth decay, broken or discolored fillings, sensitive or cracked teeth, infections and ulcers. Recent diagnostic x-rays are necessary. Please ask us about our beautiful cosmetic options as well as the helpful sedation medicines available to make your dental visit more relaxing and pleasant.

 Helpful Hints for New Patients

Please bring your dental health insurance card with you on your first visit, and a copy of any current medications you are taking.
